One page per case type, matched to the ad that sent the visitor. The form asks for state, date and accident type up front, so intake can sort a lead before anyone calls back.

A name-your-price beat store, from the direction the client picked

Clickable pages, not mockups. The client chose one of six directions, then picked from twelve takes on it. No payments are connected yet, and the page says so.
